Terms
All fees are payable in full at the time of your appointment. We
accept money orders, cashier's checks, traveler's checks, Mastercard,
Visa, American Express or Discover Card. For credit card transactions,
picture id is required. If the cardholder is not present, their written authorization
(including a copy of the cardholder's driver's license and signature) is required.
Credit card must be present.
With advance arrangement, we will also work with your health
insurance provider. We do not accept personal checks.

For your convenience we have an on-site pharmacy that can
provide you with routine post-operative antibiotics and other medications
if indicated. Medications needed for each patient vary, but we will
prepare you for your pharmacy cost when you call to schedule an
appointment. Generally, pharmacy costs range from $20-$35.
